To submit your answers to the Common Read Questions, please complete the form below by:

1. Entering your full name and email address in the spaces provided.
2. Click the 'Choose File' button to select the file on your computer which contains your answers. This can be a Word file, a PDF, a plain text file, or an RTF formatted file.
3. If you have more than one file to upload, click the 'Choose File' button again.
4. If you selected an incorrect file, you can remove it by clicking the red 'Remove File' button.
